Brookline is a Continuous Care Retirement Community that sits in a serene country setting. The campus is quiet and peaceful, with a stream that flows through the campus. Brookline offers three levels of care: Independent Living, Personal Care, and Skilled Nursing. We have shopping within a short distance, approximately 6 miles, to which we offer weekly van transportation. We have prominent and stable physicians within a 7 mile radius who offer a choice of 4 providers. Brookline is proud to offer a Continuum of Care campus to meet the needs of local seniors.

The Crossings (Independent Living) offer a true independent presence, as the cottages are not attached but are single dwelling, spacious, “homelike” residences. They are completely maintenance-free, as we have services to provide lawn maintenance, landscaping, and snow removal. Amenities of water, sewer, and trash removal are included. We schedule activities and outings weekly. Each cottage is equipped with a 24-hour Emergency Nurse Response System.

The Village (Personal Care) The Village offers spacious individual living areas with bathrooms, semi-private and private. There is 24-hour security, nurses on duty, a full calendar of activities including outings and assistance for daily care, meal preparation, and medication administration. Spacious rooms are available for family gatherings as well.

The Manor (Skilled Nursing) has 85 dually certified, licensed beds. Under the direction of resident-chosen physicians, an individualized plan of care is coordinated for each resident. It is the combination of the resident first and team approaches that contributes to our success. Our staff is dedicated and boasts many years of service, which only enhances the continuum of care for our residents.

We offer full-time, on-site therapies, specializing in short-term and long-term therapy in all three disciplines: Speech, Occupational, and Physical Therapies, plus a Certified Lymphedema Specialist. Our Therapy Department offers inpatient and outpatient and 7 day/week therapy. Specialized therapeutic treatment plans include Diathermy, ACP, Vital-Stim, Omnicycle, E-Stim, Treatment for Urinary Bladder Incontinence, CHF, COPD, CVA, Recovery from Total Joint Replacements, and Heart Surgeries.

We are centrally located between Harrisburg and State College, both of which offer great culture and diversity. Brookline is in Mifflintown, Juniata County. It is a rural and safe community with pristine settings. The beauty found in our area remains unspoiled from years gone by. The Juniata River and its tributaries are popular recreation spots, providing excellent fishing, swimming, and boating. The mountains are home to many trails for hiking and backpacking. The entertainment and cultural activities easily rival any small community, which include art exhibits, craft shows, concerts and recitals, parades, and weekly community activities. A sense of the past is ever present in our small community, and there are numerous antique and specialty shops found here.
